Child and Family Teams Overview

Sometimes parents need help. If your child is struggling with a mental health issue, educational success, a developmental delay/disability, an alcohol or drug problem, or a combination of these things you may qualify for a Child and Family Team, otherwise known as a Wraparound. The wraparound program is individualized and strength-based.  The program works to empower parents and youth to be strong advocates for their needs.

A team made up of family members, school staff, and service provider(s), who meet with you to share information, use collaborative strategies, and provide assistance to remove barriers to services that a child or family needs. The Child and Family Team process offers services in the least restrictive environment, while trying to maintain children in their most natural settings.

Wraparound meetings are a safe place for families to gain access to needed services, work collaboratively through existing barriers, and gain advice from local professionals.

How to make a referral

All Logan County families who wish to participate in a wraparound team must complete a referral (see forms) or contact our office for assistance with completion of referral paperwork.  In addition, all Logan County social service agencies, organizations, schools, and the juvenile court who work with children have the ability to make a referral to the program.


The child(ren) should meet the following criteria:

  • Custodial parent must be a resident of Logan County.

  • The child(ren) must not be over the age of 21 years.

  • The child(ren) must have needs in multiple areas of life. The Program Coordinator will determine what level of Service Coordination is needed based on the referral and circumstances.



Participation in the wraparound program is voluntary and parents have the right to opt out of services at any time.  Referrals can be faxed to (937)592-7001, emailed to, or mailed to:
